The Memphis and Shelby County Health Department can help parents meet the requirements by providing school and childcare physicals and immunizations.

If a child has not attended a Tennessee school before, the parent must show proof the child has had a physical examination. The Memphis and Shelby County Health Department provides school and childcare physicals at the following locations: Galloway Clinic, 757 Galloway; Hickory Hill Clinic, 6590 Kirby Center Cove; Southland Mall Clinic, 1215 Southland Mall (inside the Mall). For other locations, call 544-7646.

If a child is covered under TennCare, the exam is free. If the child is not covered by TennCare or other health insurance, the exam costs $85. That charge also includes any childhood immunizations that are needed.

A birth certificate is also required for children entering school. Certified copies of birth records may be obtained at the following locations: Hickory Ridge Vital Records Office, 6590 Kirby Center Cove, Suite 105; Vital Records office at the Health Department, 814 Jefferson Ave., Suite 101; or through the Vitalcheck website, http://vitalchek.com/

The short form birth certificates costs $7 per copy. Additional copies of the same record are $4 each, if purchased at the same time.
